You need to get to the toilet and maybe wash your hands.
In the plant next to you, you can find a 20p COIN. Pick it up and take the used TAMPON from the floor next to it. In the window you can find a KNIFE. Take it and in your inventory cut the string from the tampon and tie the string to the coin.
Use the coin on a string on the toilet door on the right to unlock it, then use the door to enter the toilet. Open the stall door on the right and talk to the person using the toilet. Learn about his ‘problem’ and his reluctance to leave the toilet.
Take a piece of the TOILET PAPER from the roll. In your inventory combine the piece of paper with the tampon to make it look like a joint. Talk to the man at the sink and learn about his urge. Maybe you can help him. Use the knife on his belt to lower his pants and put the tampon in his bum. Use the tampon again to put it in deeper and then talk to the man again.
Ask him if he noticed something unusual and he’ll has to admit he’s lost the urge. As thanks he’ll give you the JESTER’S JUICE. Use the drugs yourself to get into another world. On the floor, near the loudspeaker on the right is a ZAP GUN. Pick it up and then talk to the jester on the left.
The jester will let you go if you answer some questions and send someone else in your place. The correct answers to the questions are:

The jester will give you some PILLS to send the chubby replacement. Use the ladder next to the warning sign to return to the bathroom. You can try to give the pills to the man on the toilet but he won’t accept them. Instead, in your inventory, use the pills with the zap gun and then you can use the toilet. Walk south to end the game.
